    Community nursing is nursing support delivered outside of a hospital, in places like your home, supported accommodation, SIL, SDA or community setting.

    Our experienced nurses can help with day-to-day health needs, complex care, monitoring, and education for you and your support team.

    We provide community nursing for:

    • People with physical, intellectual, psychosocial and neurological disabilities
    • People living in SIL / SDA / ILO / shared or individual accommodation

    Continence assessment & support

    • Continence assessments (where applicable)

    Chronic disease management

    • Diabetes support (monitoring, insulin administration as per orders)
    • Respiratory and cardiac condition monitoring
    • Health education for participants, families and support workers

    Enteral feeding & nutrition support

    • PEG / tube feeding support as clinically appropriate
    • Feeding plans and monitoring tolerance
    • Training support workers in line with care plans

    Clinical monitoring

    • Blood pressure, pulse, oxygen saturation, BGLs
    • Early identification of health changes and escalation to GP / ED

    Health education & training

    • Training support workers in day-to-day health tasks
    • Teaching early warning signs and escalation pathways
    • Promoting independence and self-management where possible

    Some complex clinical supports may be delivered in partnership with external specialist nurses or providers, depending on NDIS registration and clinical requirements.

    • Capacity Building – Improved Daily Living / Health & Wellbeing, and/or
    • Core – Assistance with Daily Life, depending on your plan and goals.
